BRAND OVERVIEW

AlphaDay-DS is a double-strength, fixed-dose oral combination of five calcium salts of α-ketoanalogues / α-hydroxyanalogue and five essential amino acids (in free or salt form) developed for the dietary management of patients with Chronic Kidney Disease (CKD). Each film-coated tablet supplies the full complement of essential amino acid equivalents required by uraemic patients while contributing minimal nitrogenous load to the renal system.

The "DS" (Double Strength) formulation roughly doubles the per-tablet content of conventional alpha-ketoanalogue tablets, allowing fewer tablets per dose, improving patient compliance — a key concern in long-term CKD therapy where the conventional regimen often requires 9–12 tablets per day.

THERAPEUTIC INDICATION

PRIMARY INDICATION

Adjuvant nutritional therapy in adult patients with Chronic Kidney Disease (CKD) — Stages 3, 4 and 5 — receiving a low-protein diet (LPD: ~0.6 g/kg/day) or very-low-protein diet (VLPD: ~0.3–0.4 g/kg/day) to delay progression of renal failure and postpone the initiation of dialysis.

Additional indications: Diabetic nephropathy · Pre-dialysis CKD · Maintenance haemodialysis · Peritoneal dialysis · Protein-energy wasting in renal failure · Hyperphosphatemia management.

MECHANISM OF ACTION

AlphaDay-DS works on the principle of nitrogen sparing through transamination. The calcium salts of branched-chain α-ketoanalogues (and the α-hydroxyanalogue of methionine) are nitrogen-free skeletons of essential amino acids. After oral absorption, they undergo transamination in the liver and skeletal muscle:

  • Renal haemodynamics: Unlike high-protein loading, ketoanalogues do not induce glomerular hyperfiltration; they help maintain residual renal function and slow the rate of GFR decline.
  • Calcium phosphate binding: The calcium component binds dietary phosphate in the gut, providing an ancillary benefit in the management of CKD-related hyperphosphataemia and secondary hyperparathyroidism.
  • Protein synthesis support: The regenerated essential amino acids, together with the directly supplied lysine, threonine, tryptophan, histidine and tyrosine, are channelled into anabolism, preserving lean body mass on a low-protein diet.
  • Reduced uraemic load: Because the body re-uses its own nitrogen instead of producing new urea, blood urea, BUN and other uraemic toxins decline measurably.
  • Nitrogen scavenging: Endogenous waste nitrogen — mainly from the breakdown of non-essential amino acids and urea recycling — is captured to aminate the keto-skeletons, regenerating the corresponding essential amino acids (leucine, isoleucine, valine, phenylalanine, methionine).
Net clinical effect: when combined with a properly supervised LPD/VLPD, AlphaDay-DS slows progression of CKD by an estimated 30–50% versus standard care, delays the need for dialysis, mitigates protein-energy wasting and improves metabolic complications including proteinuria, calcium-phosphate dysregulation, and disturbances in lipid and carbohydrate metabolism.

DOSAGE & ADMINISTRATION

Adults (≈70 kg body weight): The usual maintenance dose is 1 tablet of AlphaDay-DS per 10 kg body weight per day, administered in 3 divided doses with main meals — typically 4 to 8 tablets per day depending on the patient's weight, GFR and dietary protein restriction. Tablets must be swallowed whole with sufficient liquid; do NOT crush, chew or break.

Initiation criterion: GFR < 25 mL/min, on a controlled LPD (≈0.6 g/kg/day) or VLPD (≈0.3–0.4 g/kg/day). Treatment is generally long-term and individualised by the treating nephrologist.
